Transcript Request Information

Transcript requests must be made in writing.  Local residents may complete a request form in
person. If that's not convenient or you're out of town, please send download the 
 
TRANSCRIPT
REQUEST FORM 
 
and mail it with payment to the address listed below.
 
An official transcript must be mailed to a school or employer; please provide all requested
information.  (We do not fax transcripts.)
 
A transcript sent or given directly to you will be marked unofficial.  A transcript is not "official"
unless it bears the embossed EHS seal and is mailed directly from the high school to the college. 
 
 
There is a $5.00 charge per transcript request, cash, check, or money order. Current
seniors are provided two transcripts at no charge.  NOTE:  Checks MUST INCLUDE current address, Driver's License number, and telephone number.
